Transösophageales interventrikuläres Delay bei Vorhofflimmern und kardialer Resynchronisation

Die transösophageale linksventrikuläre Elektrokardiographie ermöglicht die Evaluierung der elektrischen ventrikulären Desynchronisation im Rahmen der kardialen Resynchronisationstherapie der Herzinsuffizienz. Das Ziel der Untersuchung besteht in der präoperativen Abschätzung des transösophagealen interventrikulären Delays bei Vorhofflimmern und kardialer Resynchronisationstherapie. Bei Patienten mit Vorhofflimmern, Herzinsuffizienz New York Heart Association Klasse 3,0 ± 0,2 und QRS-Dauer 159,6 ± 23,9 ms wurde das fokusierte transösophageale linksventrikuläre EKG abgeleitet. Die kardiale Resynchronisationstherapie Responder QRS-Dauer korrelierte mit dem transösophagealen interventrikulären Delay bei Vorhofflimmern.
Cardiac resynchronization therapy (CRT) with biventricular pacing (BV) is an established therapy for heart failure (HF) patients with inter- and intraventricular conduction delay. The aim of this pilot study was to test the feasibility of both transesophageal measurement of left ventricular (LV) electrical delay and transesophageal LV pacing prior to implantation, to better select patients for CRT.
Cardiac resynchronisation therapy (CRT) with biventricular pacing (BV) is an established therapy for heart failure (HF) patients with interventricular conduction delay (IVCD). The aim of the study was to evaluate transesophageal IVCD and left ventricular (LV) pacing with directed electrical pacing field (EPF) in HF patients.
Methods: HF patients were analysed with bipolar transesophageal LV electrocardiogram recording and LV pacing with constant voltage stimulus output, 4 ms stimulus duration, distal cylindrical electrode (CE) and seven 6 mm hemispherical electrodes (HE) with 15 mm electrode distance (TO, Dr. Osypka, Rheinfelden, Germany).
Results: LV electrocardiogram recording with HE-HE and CE-HE evaluated a mean IVCD of 79.9 ± 36.7 ms. Directed EPF with CE-HE and HE-HE allowed LV VAT (n=12) and LV D00 pacing (n=5) with a mean effective capture output of 97.35 ± 6.64 V. In 15 responders with IVCD of 87 ± 33 ms arterial pulse pressure (PP) increased from 65 ± 24 mmHg to 79 ± 27 mmHg (p < 0.001). EPF was simulated with finite element method.
Conclusions: Transesophageal LV electrocardiography and directed EPF pacing with CE and HE allowed the evaluation of IVCD and PP to select patients for BV pacing.
Termination of atrial flutter (AFL) is not possible in all AFL patients (P) with transesophageal left atrial pacing (TLAP) with undirected electrical pacing field (EPF) and high atrial pacing threshold. Purpose of the study was to evaluate bipo-lar transesophageal left atrial electrocardiography (TLAE) and TLAP with directed EPF for evaluation and termination of AFL with and without simultaneous transesophageal echocardiography (TEE).
Methods: AFL P were analysed using either a TO electrode with one cylindrical (CE) and three or seven hemispherical electrodes (HE) or TEE electrode with four HE (Osypka, Rheinfelden, Germany). Burst TLAP cycle length was between 200msand 50ms.
Results: AFL cycle length was 233±30 ms with mean ventricular cycle length of 540±149 ms. AFL could be terminated by rapid bipolar TLAP with directed EPF using HE-HE and CE-HE with induction of atrial fibrillation (AF), induction of AF and spontaneous conversion to sinus rhythm and direct conversion to sinus rhythm. Directed EPF was simulated with finite element method.
Conclusions: AFL can be evaluated by bipolar TLAE. AFL can be terminated with rapid TLAP with directed EPF with and without simultaneous TEE. Bipolar TLAE with rapid TLAP is a safe, simple and useful method for evaluation and termination of AFL.
Das Ausmaß der elektrischen ventrikulären Desynchronisation bei reduzierter linksventrikulärer Funktion ist von Bedeutung für den Erfolg der Resynchronisationstherapie der Herzinsuffizienz mit biventrikulärer Stimulation. Das Ziel der Untersuchung besteht in der nichtinvasiven Messung der elektrischen inter-ventrikulären Desynchronisation mit und ohne ischämische Herzerkrankung bei kardialen Resynchronisationstherapie Respondern. Bei Patienten mit 25,3 ± 7,3 % reduzierter linksventrikulärer Ejektionsfraktion und 166,9 ± 38,5 ms QRS-Dauer wurde das transösophageale linksventrikuläre EKG abgeleitet. Die QRS-Dauer korrelierte mit dem interventrikulären und links-ventrikulären Delay bei Resynchronisationstherapie Respondern mit nicht-ischämischer Herzerkrankung.
Die vorliegende Erfindung betrifft Vorrichtungen zum Überwachen und Optimieren einer zeitlichen Triggerstabilität einer extrakorporalen Kreislaufunterstützung sowie Steuer- und Regeleinheiten zur extrakorporalen Kreislaufunterstützung, umfassend eine solche Vorrichtung und entsprechende Verfahren. Entsprechend wird eine Vorrichtung (10) zum Überwachen einer zeitlichen Triggerstabilität einer extrakorporalen Kreislaufunterstützung vorgeschlagen, welche dazu eingerichtet ist, einen ersten Datensatz (14) einer Messung eines EKG-Signals eines unterstützten Patienten über einen vorgegebenen Zeitraum zu empfangen. Die Vorrichtung (10) umfasst eine Auswerteeinheit (16), welche dazu eingerichtet ist, mehrere R-Trigger (26) aus dem ersten Datensatz (14) zu bestimmen oder zu identifizieren, wobei die Auswerteeinheit (16) weiterhin dazu eingerichtet ist, einen zweiten Datensatz (20) mit ausgewerteten EKG-Signalen und mehreren R-Triggern (28) zu empfangen oder bereitzustellen und den zweiten Datensatz (20) selektiv auf dem ersten Datensatz (14) abzubilden. Die Vorrichtung ist weiterhin dazu eingerichtet, ein Signal (22) auszugeben, welches kennzeichnend für einen zeitlichen Abstand sukzessiver R-Trigger (26) aus dem ersten Datensatz (14) und darauf abgebildeten sukzessiven R-Trigger (28) aus dem zweiten Datensatz (20) ist.

The present invention relates to devices for monitoring and optimising a temporal trigger stability of an extracorporeal circulatory support means, and to open-loop and closed-loop control units for the extracorporeal circulatory support means comprising such a device, and to corresponding methods. A device (10) for monitoring a temporal trigger stability of an extracorporeal circulatory support means is accordingly proposed, which device is designed to receive a first dataset (14) of a measurement of an ECG signal of a supported patient over a predefined period of time. The device (10) comprises an evaluation unit (16), which is designed to determine or identify a plurality of R triggers (26) from the first dataset (14), wherein the evaluation unit (16) is also designed to receive or provide a second dataset (20) having evaluated ECG signals and a plurality of R triggers (28) and to selectively map the second dataset (20) on the first dataset (14). The device is also designed to emit a signal (22) that characterises a temporal gap between successive R triggers (26) from the first dataset (14) and successive R triggers (28) from the second dataset (20) which are mapped on the first dataset.

The open-loop and closed-loop control unit (10) is further configured to output an open-loop and/or closed-loop signal (16) for extracorporeal circulatory support at a predefined point in time after the at least one amplitude change (14).

Background: The electrical field (E-field) of the biventricular (BV) stimulation is important for the success of cardiac resynchronization therapy (CRT) in patients with cardiac insufficiency and widened QRS complex. The 3D modeling allows the simulation of CRT and high frequency (HF) ablation.
Purpose: The aim of the study was to model different pacing and ablation electrodes and to integrate them into a heart model for the static and dynamic simulation of atrial and BV stimulation and high frequency (HF) ablation in atrial fibrillation (AF).
Methods: The modeling and simulation was carried out using the electromagnetic simulation software CST (CST Darmstadt). Five multipolar left ventricular (LV) electrodes, one epicardial LV electrode, four bipolar right atrial (RA) electrodes, two right ventricular (RV) electrodes and one HF ablation catheter were modeled. Selected electrodes were integrated into the Offenburg heart rhythm model for the electrical field simulation. The simulation of an AV node ablation at CRT was performed with RA, RV and LV electrodes and integrated ablation catheter with an 8 mm gold tip.
Results: The right atrial stimulation was performed with an amplitude of 1.5 V with a pulse width of 0.5. The far-field potentials generated by the atrial stimulation were perceived by the right and left ventricular electrode. The far-field potential at a distance of 1 mm from the right ventricular electrode tip was 36.1 mV. The far-field potential at a distance of 1 mm from the left ventricular electrode tip was measured with 37.1 mV. The RV and LV stimulation were performed simultaneously at amplitude of 3 V at the LV electrode and 1 V at the RV electrode with a pulse width of 0.5 ms each. The far-field potentials generated by the BV stimulations could be perceived by the RA electrode. The far-field potential at the RA electrode tip was 32.86 mV. AV node ablation was simulated with an applied power of 5 W at 420 kHz and 10 W at 500 kHz at the distal 8 mm ablation electrode.
Conclusions: Virtual heart and electrode models as well as the simulations of electrical fields and temperature profiles allow the static and dynamic simulation of atrial synchronous BV stimulation and HF ablation at AF. The 3D simulation of the electrical field and temperature profile may be used to optimize the CRT and AF ablation.
Cardiac resynchronization therapy with atrioventricular and interventricular pacing delay optimized biventricular pacing is an established therapy for heart failure patients with sinus rhythm and reduced left ventricular ejection fraction. The aim of the study was to evaluate atrioventricular and interventricular pacing delay optimization in cardiac resynchroniza-tion therapy by transthoracic impedance cardiography in biventricular pacing with different left ventricular electrode po-sition. In biventricular pacing heart failure patients with lateral, posterolateral and anterolateral left ventricular electrode position, the mean optimal atrioventricular sening delay was 108.6 ± 20.3 ms and the mean optimal interventricular pac-ing delay -12.3 ± 25.9 ms. Transthoracic impedance cardiography may be a useful technique to optimize atrioventricular and interventricular pacing delay in biventricular pacing with different left ventricular electrode position.
Hintergrund: Das elektrische interventrikuläre Delay (IVD) und die Lage der linksventrikulären (LV) Elektrode zum Ort der spätesten LV Erregung sind bei Patienten (P) mit Herzinsuffizienz (HF), reduzierter LV Funktion und breiter QRS Dauer (QRSD) von Bedeutung für den Erfolg der kardialen Resynchronisationstherapie (CRT). Die LV Elektrokardiographie ermöglicht eine Abschätzung des elektrischen IVD. Ziel der Studie besteht in der nicht-invasiven Evaluierung des elektrischen IVD bei Patienten (P) mit Vorhofflimmern (AFib) mit und ohne CRT mit biventrikulärer (BV) Stimulation.
Methoden: Bei 49 HF P mit AFib (Alter 63,9 ± 10,8 Jahre; 43 Männer und 6 Frauen) mit New York Heart Association (NYHA) Klasse 2,9 ± 0,4, LV Ejektionsfraktion 26,03 ± 7,99 % und QRS-Dauer (QRSD) 143,69 ± 35,62 ms wurde das elektrische IVD als Intervall zwischen Beginn des QRS-Komplexes im Oberflächen EKG und Beginn des LV Signals im transösophagealen LV EKG bei 31 HF P mit AFib und bei 18 HF P mit AFib und CRT präoperativ bestimmt. Das fokussierte bipolare LV EKG wurde mittels Osypka TO Sonde mit halbkugelförmigen Elektroden in Höhe des maximalen LV Signals registriert.
Ergebnisse: Bei 31 HF P mit AFib betrugen QRSD 135,48 ± 38,78 ms, IVD 49,55 ± 26,38 ms, QRSD-IVD-Verhältnis 3,12 ± 1,11 und das IVD korrelierte mit der QRSD (r=0,75, P<0,001) und dem QRSD-IVD-Verhältnis (r=-0,67, P<0,001) (Fig.). Bei 18 HF P mit AFib und CRT Defibrillator betrugen QRSD 157,83 ± 24,38 ms, IVD 61,94 ± 26,88 ms, QRSD-IVD-Verhältnis 3,12 ± 1,89 und das IVD korrelierte mit der QRSD (r=0,47, P=0,049) und dem QRSD-IVD-Verhältnis (r=-0,73, P<0,001). Bei 72,2 % CRT Responder (R) (n=13) betrugen QRSD 158,15 ± 22,4 ms, IVD 64,23 ± 24,62 ms, QRSD-IVD-Verhältnis 2,82 ± 1,32 und das IVD korrelierte mit der QRSD (r=0,57, P=0,043) und dem QRSD-IVD-Verhältnis (r=-0,76, P=0,0024). Bei 27,8 % CRT Non-Responder (NR) (n=5) betrugen QRSD 157 ± 31,94 ms, IVD 56 ± 34,52 ms, QRSD-IVD-Verhältnis 3,88 ± 2,98 und das IVD korrelierte nicht mit der QRSD (r=0,33, P=0,591) und dem QRSD-IVD-Verhältnis (r=-0,732, P=0,159). Die CRT R verbesserten sich in der NYHA Klasse von 3 ± 0,2 auf 2,2 ± 0,3 (P<0,001) während 15,3 ± 13,1 Monaten BV Stimulation. Bei 15 CRT NR kam es zu keiner Verbesserung der NYHA Klasse von 3 auf 3,3 ± 0,97 (P=0,529) während 18,8 ± 20,7 Monaten BV Stimulation.
Schlussfolgerungen: Das transösophageale LV EKG ermöglicht bei HF-P mit AFib die nichtinvasive Messung des elektrischen IVD präoperativ vor CRT. IVD und QRSD-IVD-Verhältnis sind möglicherweise einfach anwendbare Parameter zur Vorhersage von CRT R und CRT NR bei P mit AFib.
Background: Cardiac resynchronization therapy (CRT) with biventricular (BV) pacing is an established therapy for heart failure (HF) patients (P) with sinus rhythm, reduced left ventricular (LV) ejection fraction (EF) and electrical ventricular desynchronization. The aim of the study was to evaluate electrical interventricular delay (IVD) and left ventricular delay (LVD) in right ventricular (RV) pacemaker pacing before upgrading to CRT BV pacing.
Methods: HF P (n=11, age 69.0 ± 7.9 years, 1 female, 10 males) with DDD pacemaker (n=10), DDD defibrillator (n=1), RV pacing, New York Heart Association (NYHA) class 3.0 ± 0.2 and 24.5 ± 4.9 % LVEF were measured by surface ECG and transesophageal bipolar LV ECG before upgrading to CRT defibrillator (n=8) and CRT pacemaker (n=3). IVD was measured between onset of QRS in the surface ECG and onset of LV signal in the transesophageal ECG. LVD was measured between onset and offset of LV signal in the transesophageal ECG. CRT atrioventricular (AV) and BV pacing delay were optimized by impedance cardiography.
Results: Interventricular and intraventricular desynchronization in RV pacemaker pacing were 228.2 ± 44.8 ms QRS duration, 86.5 ± 32.8ms IVD, 94.4 ± 23.8ms LVD, 2.6 ± 0.8 QRS-IVD-ratio with correlation between IVD and QRS-IVD-ratio (r=-0.668 P=0.0248) and 2.3 ± 0.7 QRS-LVD-ratio. The LVEF-IVD-ratio was 0.3 ± 0.1 with correlation between IVD and LVEF-IVD-ratio (r=-0.8063 P=0.00272) and with correlation between QRS duration and LVEF-IVD-ratio (r=-0.7251 P=0.01157). Optimal sensing and pacing AV delay were 128.3 ± 24.8 ms AV delay after atrial sensing (n=6) and 173.3 ± 40.4 ms AV delay after atrial pacing (n=3). Optimal BV pacing delay was -4.3 ± 11.3 ms between LV and RV pacing (n=7). During 30.4 ± 29.6 month CRT follow-up, the NYHA class improved from 3.1 ± 0.2 to 2.2 ± 0.3.
Conclusions: Transesophageal electrical IVD and LVD in RV pacemaker pacing may be additional useful ventricular desynchronization parameters to improve P selection for upgrading RV pacemaker pacing to CRT BV pacing.
Cardiac resynchronization therapy is an established therapy for heart failure patients with sinus rhythm, reduced left ventricular ejection fraction and prolongation of QRS duration. The aim of the study was to evaluate ventricular desynchronization with electrical interventricular delay (IVD) to left ventricular delay (LVD) ratio in atrial fibrillation heart failure patients. IVD and LVD were measured by transesophageal posterior left ventricular ECG recording. In atrial fibrillation heart failure patients with prolonged QRS duration, the mean IVD-to-LVD-ratio was 0.84 +/- 0.42 with a range from 0.17 to 2.2 IVD-to-LVD-ratio. IVD-to-LVD-ratio correlated with QRS duration. IVD-to-LVD-ratio may be a useful parameter to evaluate electrical ventricular desynchronization in atrial fibrillation heart failure patients.
Cardiac resynchronization therapy (CRT) is an established class I level A biventricular pacing therapy in chronic heart failure patients with left bundle branch block and reduced left ventricular ejection fraction, but not all patients improved clinically. Purpose of the study was to evaluate electrical interatrial conduction delay (IACD) to interventricular conduction delay (IVCD) ratio with focused transesophageal left atrial and left ventricular electrocardiography.
Methods: Thirty eight chronic heart failure patients (age 63.4±10.2 years; 3 females, 35 males) with New York Heart Association (NYHA) functional class 3.0±0.2 and 171.71±36.17ms QRS duration were analysed using posterior left atrial and left ventricular transesophageal electrocardiography with hemispherical electrodes before CRT. Electrical IACD was measured between onset of P-wave in the surface ECG and onset of left atrial signal. Electrical IVCD was measured between onset of QRS complex in the surface ECG and onset of left ventricular signal.
Results: Electrical IACD and IVCD could be evaluated by transesophageal left atrial and left ventricular electrocardiography in all heart failure patients with correlation to 1.18±0.92 IACD-IVCD-ratio (r=-0.57, P<0.001; r=0.66, P<0.001). There were 32 CRT responder with reduction of NYHA class from 3.0±0.22 to 1.97±0.31 (P<0.001) during 16.5±18.9 month CRT with 75.19±33.49ms IACD, 78.91±24.73ms IVCD, 1.04±0.66 IACD-IVCD-ratio and correlation between IACD and IACDIVCD- ratio (r=0.84, P<0.001). There were 6 CRT nonresponder with no reduction of NYHA class from 3.0±0.3 to 2.9±0.5 during 14.3±13.7 month biventricular pacing, 50.0±28.26ms IVCD (P=0.014), 1.92±1.65 IACD-IVCD-ratio (P=0,029) and correlation between 67.0±24.9ms IACD and IACD-IVCD-ratio (r=0.85, P=0.031).
Conclusions: Focused transesophageal left atrial and left ventricular electrocardiography can be utilized to analyse electrical IACD and IVCD in heart failure patients. IACDIVDC- ratio may be a useful parameter to evaluate electrical left cardiac desynchronization in heart failure patients.
New employees are supposed to quickly understand their tasks, internal processes and familiarize with colleagues. This process is called “onboarding” and is still mainly realized by organizational methods from human resource management, such as introductory events or special employee sessions. Software tools and especially mobile applications are an innovative means to support provide onboarding processes in a modern, even remote, way. In this paper we analyze how the use of gamification can enhance onboarding processes. Firstly, we describe a mobile onboarding application specifically developed for the young, technically literate generations Y and Z, who are just about to start their career. Secondly, we report on a study with 98 students and young employees. We found that participants enjoyed the gamified application. They especially appreciated the feature “Team Bingo” which facilitates social integration and teambuilding. Based on the OCEAN personality model (“Big Five”), the personality traits agreeableness and openness revealed significant correlations with a preference for the gamified onboarding application.
The present work ties in with the problem of bicycle road assessment that is currently done using expensive special measuring vehicles. Our alternative approach for road condition assessment is to mount a sensor device on a bicycle which sends accelerometer and gyroscope data via WiFi to a classification server. There, a prediction model determines road type and condition based on the sensor data. For the classification task, we compare different machine learning methods with each other, whereby validation accuracies of 99% can be achieved with deep residual networks such as InceptionTime. The main contribution of this work with respect to comparable work is that we achieve excellent accuracies on a realistic dataset classifying road conditions into nine distinct classes that are highly relevant for practice.

Phosphate-based inorganic–organic hybrid nanoparticles (IOH-NPs) with the general composition [M]2+[Rfunction(O)PO3]2– (M = ZrO, Mg2O; R = functional organic group) show multipurpose and multifunctional properties. If [Rfunction(O)PO3]2– is a fluorescent dye anion ([RdyeOPO3]2–), the IOH-NPs show blue, green, red, and near-infrared fluorescence. This is shown for [ZrO]2+[PUP]2–, [ZrO]2+[MFP]2–, [ZrO]2+[RRP]2–, and [ZrO]2+[DUT]2– (PUP = phenylumbelliferon phosphate, MFP = methylfluorescein phosphate, RRP = resorufin phosphate, DUT = Dyomics-647 uridine triphosphate). With pharmaceutical agents as functional anions ([RdrugOPO3]2–), drug transport and release of anti-inflammatory ([ZrO]2+[BMP]2–) and antitumor agents ([ZrO]2+[FdUMP]2–) with an up to 80% load of active drug is possible (BMP = betamethason phosphate, FdUMP = 5′-fluoro-2′-deoxyuridine 5′-monophosphate). A combination of fluorescent dye and drug anions is possible as well and shown for [ZrO]2+[BMP]2–0.996[DUT]2–0.004. Merging of functional anions, in general, results in [ZrO]2+([RdrugOPO3]1–x[RdyeOPO3]x)2– nanoparticles and is highly relevant for theranostics. Amine-based functional anions in [MgO]2+[RaminePO3]2– IOH-NPs, finally, show CO2 sorption (up to 180 mg g–1) and can be used for CO2/N2 separation (selectivity up to α = 23). This includes aminomethyl phosphonate [AMP]2–, 1-aminoethyl phosphonate [1AEP]2–, 2-aminoethyl phosphonate [2AEP]2–, aminopropyl phosphonate [APP]2–, and aminobutyl phosphonate [ABP]2–. All [M]2+[Rfunction(O)PO3]2– IOH-NPs are prepared via noncomplex synthesis in water, which facilitates practical handling and which is optimal for biomedical application. In sum, all IOH-NPs have very similar chemical compositions but can address a variety of different functions, including fluorescence, drug delivery, and CO2 sorption.

In the field of neuroprosthetics, the current state-of-the-art method involves controlling the prosthesis with electromyography (EMG) or electrooculography/electroencephalography (EOG/EEG). However, these systems are both expensive and time consuming to calibrate, susceptible to interference, and require a lengthy learning phase by the patient. Therefore, it is an open challenge to design more robust systems that are suitable for everyday use and meet the needs of patients. In this paper, we present a new concept of complete visual control for a prosthesis, an exoskeleton or another end effector using augmented reality (AR) glasses presented for the first time in a proof-of-concept study. By using AR glasses equipped with a monocular camera, a marker attached to the prosthesis is tracked. Minimal relative movements of the head with respect to the prosthesis are registered by tracking and used for control. Two possible control mechanisms including visual feedback are presented and implemented for both a motorized hand orthosis and a motorized hand prosthesis. Since the grasping process is mainly controlled by vision, the proposed approach appears to be natural and intuitive.
Die Erfindung betrifft ein Verfahren zur Steuerung eines Geräts, insbesondere einer Handprothese oder eines Roboterarms, wobei wenigstens ein an oder im Bezug zu dem Gerät positionierter Marker von einer an einer Bedienperson angeordneten Kamera erkannt wird, wobei ab dem Erkennen des wenigstens einen Markers eine vordefinierte Bewegung der Bedienperson zusammen mit der Kamera erkannt wird und zum Auslösen einer entsprechenden Aktion des Geräts verwendet wird, wobei die vordefinierte Bewegung einer Bedienperson in Form eines Sehstrahls mittels Kamera-Tracking erkannt wird. Weiterhin betrifft die Erfindung eine Anordnung aus einem Gerät, insbesondere einer Handprothese oder eines Roboterarms, und einer AR-Brille zur Durchführung eines derartigen Verfahrens.
Restoring hand motion to people experiencing amputation, paralysis, and stroke is a critical area of research and development. While electrode-based systems that use input from the brain or muscle have proven successful, these systems tend to be expensive and di¨cult to learn. One group of researchers is exploring the use of augmented reality (AR) as a new way of controlling hand prostheses. A camera mounted on eyeglasses tracks LEDs on a prosthetic to execute opening and closing commands using one of two different AR systems. One system uses a rectangular command window to control motion: crossing horizontally signals “open” along one direction and “close” in the opposite direction. The second system uses a circular command window: once control is enabled, gripping strength can be controlled by the direction of head motion. While the visual system remains to be tested with patients, its low cost, ease of use, and lack of electrodes make the device a promising solution for restoring hand motion.
A new concept for robust non-invasive optical activation of motorized hand prostheses by simple and non-contactcommands is presented. In addition, a novel approach for aiding hand amputees is shown, outlining significantprogress in thinking worth testing. In this, personalized 3D-printed artificial flexible hands are combined withcommercially available motorized exoskeletons, as they are used e.g. in tetraplegics.
Background: This paper presents a conceptual design for an anthropomorphic replacement hand made of silicone that integrates a sensory feedback system. In combination with a motorized orthosis, it allows performing movements and registering information on the flexion and the pressure of the fingers.
Methods: To create the replacement hand, a three-dimensional (3D) scanner was used to scan the hand of the test person. With computer-aided design (CAD), a mold was created from the hand, then 3D-printed. Bending and force sensors were attached to the mold before silicone casting to implement the sensory feedback system. To achieve a functional and anthropomorphic appearance of the replacement hand, a material analysis was carried out. In two different test series, the properties of the used silicones were analyzed regarding their mechanical properties and the manufacturing process.
Results: Individual fingers and an entire hand with integrated sensors were realized, which demonstrated in several tests that sensory feedback in such an anthropomorphic replacement hand can be realized. Nevertheless, the choice of silicone material remains an open challenge, as there is a trade-off between the hardness of the material and the maximum mechanical force of the orthosis.
Conclusion: Apart from manufacturing-related issues, it is possible to cost-effectively create a personalized, anthropomorphic replacement hand, including sensory feedback, by using 3D scanning and 3D printing techniques.
A complete thermomechanical fatigue (TMF) life prediction methodology is developed for predicting the TMF life of cast iron cylinder heads for efficient heavy duty internal combustion engines. The methodology uses transient temperature fields as thermal loads for the non-linear structural finite-element analysis (FEA). To obtain reliable stress and strain histories in the FEA for cast iron materials, a time and temperature dependent plasticity model which accounts for viscous effects, non-linear kinematic hardening and tensioncompression asymmetry is required. For this purpose a unified elasto-viscoplastic Chaboche model coupled with damage is developed and implemented as a user material model (USERMAT) in the general purpose FEA program ANSYS. In addition, the mechanismbased DTMF model for TMF life prediction developed in Part I of the paper is extended to three-dimensional stress states under transient non-proportional loading conditions. The material properties of the plasticity model are determined for lamellar graphite cast iron GJL250 and vermicular graphite cast iron GJV450 from isothermal and non-isothermal uniaxial tests. The methodology is applied to obtain a TMF life prediction on two cast iron cylinder heads for heavy duty diesel engine applications made from both cast iron materials. It is shown that the life predictions using the developed methodology correlate very well with observed lives from two bench tests in terms of location as well as number of cycles to failure.
High temperature components in internal combustion engines and exhaust systems must withstand severe mechanical and thermal cyclic loads throughout their lifetime. The combination of thermal transients and mechanical load cycling results in a complex evolution of damage, leading to thermomechanical fatigue (TMF) of the material. Analytical tools are increasingly employed by designers and engineers for component durability assessment well before any hardware testing. The DTMF model for TMF life prediction, which assumes that micro-crack growth is the dominant damage mechanism, is capable of providing reliable predictions for a wide range of high-temperature components and materials in internal combustion engines. Thus far, the DTMF model has employed a local approach where surface stresses, strains, and temperatures are used to compute damage for estimating the number of cycles for a small initial defect or micro-crack to reach a critical length. In the presence of significant gradients of stresses, strains, and temperatures, the use of surface field values could lead to very conservative estimates of TMF life when compared with reported lives from hardware testing. As an approximation of gradient effects, a non-local approach of the DTMF model is applied. This approach considers through-thickness fields where the micro-crack growth law is integrated through the thickness considering these variable fields. With the help of software tools, this method is automated and applied to components with complex geometries and fields. It is shown, for the TMF life prediction of a turbocharger housing, that the gradient correction using the non-local approach leads to more realistic life predictions and can distinguish between surface cracks that may arrest or propagate through the thickness and lead to component failure.
Cast aluminum cylinder blocks are frequently used in gasoline and diesel internal combustion engines because of their light-weight advantage. However, the disadvantage of aluminum alloys is their relatively low strength and fatigue resistance which make aluminum blocks prone to fatigue cracking. Engine blocks must withstand a combination of low-cycle fatigue (LCF) thermal loads and high-cycle fatigue (HCF) combustion and dynamic loads. Reliable computational methods are needed that allow for accurate fatigue assessment of cylinder blocks under this combined loading. In several publications, the mechanism-based thermomechanical fatigue (TMF) damage model DTMF describing the growth of short fatigue cracks has been extended to include the effect of both LCF thermal loads and superimposed HCF loadings. This approach is applied to the finite life fatigue assessment of an aluminum cylinder block. The required material properties related to LCF are determined from uniaxial LCF tests. The additional material properties required for the assessment of superimposed HCF are obtained from the literature for similar materials. The predictions of the model agree well with engine dyno test results. Finally, some improvements to the current process are discussed.
Im Kern dieser Arbeit geht es um das Begreifen von Kognition. Der Kognitionsbegriff wird zur Schlüsselkategorie in den basalen Gedanken- bzw. Modellgebäuden und den daraus entwickelten Algorithmen. Es ist eine Arbeit, die unter anderem die philosophischen Positionen des Reduktionismus, Funktionalismus und Konstruktivismus mit einer kognitiven Theorie so in Verbindung bringt, um diese erkenntnistheoretischen Ismen mit den Erkenntnissen einer technologisierten Kognitionswissenschaft zu synchronisieren und als algorithmisierte Theorie im Rahmen eines Entwicklungsprojekt als artifizielle Kognition zu realisieren. Die Arbeit ist somit theoretisch fundiert und praktisch orientiert.
Instruktionale Texte und Lernvideos – Konzeption und Evaluation zweier multimedialer Lernformate

Bei der Konzeption von digitalen Lernmedien sind von Seiten der Entwickler viele Entscheidungen hinsichtlich der Präsentation von Lerninhalten zu treffen, was zum einen die mediale Darstellung und zum anderen die didaktische Aufbereitung der fachlichen Inhalte betrifft. Im vorliegenden Text werden zwei digitale Lernmedien auf der Basis von Einschätzungen von Studierenden wirtschaftswissenschaftlicher und technischer Studiengänge an den Hochschulstandorten Offenburg und Pforzheim analysiert. Die Konzepte der beiden Hochschulen unterscheiden sich deutlich voneinander. Während in Offenburg Lernvideos mit Audiokommentar Verwendung finden, wird in Pforzheim mit statischen instruktionalen Texten und obligatorischen Single-Choice-Übungsaufgaben in einer Moodle-Lernumgebung gearbeitet. Aus Sicht des Aufforderungscharakters, also der Motivation, sich mit den entsprechenden Materialien zu beschäftigen, werden sowohl die instruktionalen Texte als auch die Lernvideos von den Studierenden geschätzt. Es zeigt sich weiterhin, dass Studierende die Lernvideos mit einer gewissen Präferenz lieber allein als zu zweit bearbeiten, während die Bearbeitung der statischen Texte differenzierter betrachtet werden muss. Fokussierende Fragen bewerten nahezu alle Studierenden als lernförderlich für ihren Lernprozess. Allerdings finden sich Hinweise darauf, dass fokussierende Fragen von leistungsschwächeren im Vergleich zu leistungsstärkeren Studierenden als weniger hilfreich angesehen werden. Ebenso lässt sich die Hypothese aufstellen, dass manche Studierende sowohl auf die Unterstützung eines Lernpartners/einer Lernpartnerin als auch auf lernförderliche Fragen verzichten möchten, um ihren Lernprozess möglichst autonom organisieren zu können.
Android is an operating system which was developed for use in smart mobile phones and is the current leader in this market. A lot of efforts are being spent to make Android available to the embedded world, as well. Many embedded systems do not have a local GUI and are therefore called headless devices. This paper presents the results of an analysis of the general suitability of Anroid in headless embedded systems and ponders the advantages and disadvantages. It focuses on the hardware related issues, i.e. to what extent Android supports hardware peripherals normally used in embedded systems.
Cardiac resynchronization therapy with atrioventricular and interventricular delay optimized biventricular pacing is an established therapy for symptomatic heart failure patients with prolongation of QRS duration, left bundle branch block and reduced left ventricular ejection fraction. The aim of the investigation was to evaluate right atrial, right ventricular and left ventricular electrical signals of implantable electronic cardiac devices with and without signal averaging technique with novel LabVIEW software. Electrical interatrial conduction delay and inter-ventricular conduction delay may be useful parameters to evaluate electrical atrial and ventricular desynchronization in heart failure patients.
Vulnerabilitätsanalyse "Hitzestress und menschliche Gesundheit" am Beispiel der Stadt Reutlingen

In diesem Modellprojekt wird das Schutzgut "Menschliche Gesundheit" insbesondere unter dem Gesichtspunkt der im Rahmen des globalen Klimawandels zu erwartenden Überhitzung der Städte ("städtische Hitzeinseln") betrachtet.
In der Großstadt Reutlingen ("Tor zur Schwäbischen Alb/112.500 EW) mit ihrer Pfortenlage am Rande der Schwäbischen Alb und der Höhenlage (400-800 m) sowie der Bebauungsdichte werden bis 2050 bzw. 2100 (Strategie zur Anpassung an den Klimawandel Baden-Württemberg - Vulnerabilitäten und Klimaanpassungsmaßnahmen, 2015) die massivsten Auswirkungen bezüglich Aufenthaltsbehaglichkeit und Gesundheitsfolgen in Reutlingen erwartet.
Der Untersuchungsschwerpunkt liegt im Wirkungsbereich Mensch-Siedlung, d.h. in der Betrachtung von empfindlichen Bevölkerungspopulationen (z.B. ältere Menschen) und hitzeempfindlichen Nutzungsstrukturen (z.B. verdichteten städtischen Siedlungsflächen). Insbesondere die bereits in der abgeschlossenen Gesamtstädtischen Klimaanalyse ermittelten überwärmten Areale ("hot spots") und die im Rahmen des Klimawandels für 2020-2050 zukünftig zu erwartende Hitzestressbelastung bei empfindlichen Bevölkerungsgruppen in Stadtquartieren und Funktionsbauten, stehen im Zenit der Untersuchung.
Dabei wird über das Kriterium Empfindlichkeit (Basis sind z.B. quartierbezogene Datenstrukturen von Älteren, Einrichtungen wie Krankenhäuser, Kinderpflegeeinrichtungen, Alten- Behinderten- und Pflegeheime) die zukünftige Hitzestress-Belastung für Reutlingen erarbeitet. Weiteres wichtiges Kriterium ist die Betroffenheit nach Standortsituation (Höhenlage, Durchlüftungsverhältnisse, Bioklima/PMV = Maß für die bioklimatische Behaglichkeit) und die Anzahl hitzestressgeplagter Menschen (Kinder, Kranke, Ältere). Insbesondere für das Szenarium 2020 bis 2050 (s. Strategie zur Anpassung an den Klimawandel Baden-Württemberg - Vulnerabilitäten und Klimaanpassungsmaßnahmen, 2015) werden objekt- bzw. einrichtungsbezogen (z.B. Altenpflegeeinrichtungen) sowie quartiersspezifisch (Stadtstrukturtypen) die Auswirkungen bzw. Verwundbarkeiten erarbeitet. Dieser objektspezifische (bauklimatische) Ansatz, die innovative Indikatorenbildung zur situativen kommunalen Anwendbarkeit auch über Reutlingen hinaus sowie der partizipative Ansatz mit Nichtregierungsorganisationen (NGO´s) begründet den Modellcharakter ("Reutlinger Modell") dieser Untersuchung. Das Modellprojekt bildet das zweite Modul in einem dreiteiligen Klimaanpassungskonzept für die Stadt Reutlingen.
Pressure dynamics in metal-oxygen (metal-air) batteries: a case study on sodium superoxide cells

Electrochemical reactions in metal–oxygen batteries come along with the consumption or release of gaseous oxygen. We present a novel methodology for investigating electrode reactions and transport phenomena in metal–oxygen batteries by measuring the pressure dynamics in an enclosed gas reservoir above the oxygen electrode. The methodology is exemplified by a room-temperature sodium–oxygen battery forming sodium superoxide (NaO2) in an electrolyte of diethylene glycol dimethyl ether (diglyme) and sodium trifluoromethanesulfonate (NaOSO2CF3, NaOTf). The experiments are supported by microkinetic simulations with a one-dimensional multiphysics continuum model. During galvanostatic cycling over 30 cycles, a constant oxygen consumption/release rate is observed upon discharge/charge. The number of transferred electrons per oxygen molecule is calculated to 1.01 ± 0.02 and 1.03 ± 0.02 for discharge and charge, respectively, confirming the nature of the oxygen reaction product as superoxide O2–. The same ratio is observed in cyclic voltammetry experiments with low scan rate (<1 mV/s). However, at higher scan rates, the ratio increases as a result of oxygen transport limitations in the electrolyte. We introduce electrochemical pressure impedance spectroscopy (EPIS) for simultaneously analyzing current, voltage, and pressure of electrochemical cells. Pressure recording significantly increases the sensitivity of impedance toward oxygen transport properties of the porous electrode systems. In addition, we report experimental data on the diffusion coefficient and solubility of oxygen in electrolyte solutions as important parameters for the microkinetic models.
Die Digitalisierung kann der Türöffner sein, um effizient die mittelständische Industrie und den Energiemarkt zu verbinden. Das Projekt GaIN hat das Ziel, mit hochaufgelösten Produktions- und Messdaten von zehn mittelständischen Industriebetrieben neuartige Tarife und angepasste Marktplattformen zu entwickeln, die Prognosegüte für Energiebedarf, Nachfrage und Flexibilitätsverfügbarkeit zu erhöhen, die Interaktion vieler flexibler Unternehmen im Verteilnetz und in dem Bilanzkreis zu bewerten und die Auswirkung einer Nutzung der Daten auf die Energiewende anhand einer Systemanalyse zu beurteilen.
Ziel des Pilotprojektes EnMa-HAW ist die Erarbeitung und Erprobung technisch und organisatorisch übertragbarer Konzepte für ein automationsgestütztes Energiemanagement an allen Hochschulen für angewandte Wissenschaften im Land Baden-Württemberg. Das Energiemanagement wird technisch mittels Messtechnik, Datenerfassung, Datenspeicherung und Visualisierung umgesetzt und organisatorisch mit einem Energiezirkel in den Hochschulen verankert.
Auswirkung eines Importstopps russischer Energieträger auf die Klimaschutzziele in Deutschland

Ein Importstopp russischer Energieträger nach Deutschland wird derzeit vermehrt diskutiert. Wir wollen die Diskussion unterstützen, indem wir einen Weg zeigen, wie das Elektrizitätssystem in Deutschland kurzfristig mit geringen Energieimporten auskommt und welche Maßnahmen notwendig sind, um die Klimaschutzziele trotzdem einzuhalten. Die Ergebnisse eines solchen Energiewendeszenarios mit reduzierter Importabhängigkeit werden mit dem Energiesystemmodell MyPyPSA-Ger berechnet. Die wichtigsten Erkenntnisse sind, dass ein zügiger Ausbau Erneuerbarer Energien und von Speichertechnologien • die Abhängigkeit des deutschen Elektrizitätssystems von Energieimporten deutlich reduziert. • auch langfristig keine wesentlichen Importe der Energieträger Erdgas, Steinkohle und Mineralöl nach sich zieht. • über die Klimaziele der Bundesregierung hinaus das 1,5-Grad-Ziel im Elektrizitätssystem erreicht wird.
